【託児サービス利用における留意事項】
1. お子さんの健康状態の確認について
・本託児サービスでは、安全に保育を行うために、託児サービスを利用するお子さんの当日の健康状態を確認した上で、お預かりします。
・ご利用は生後２ヶ月～小学生までの健康なお子さまを対象とさせていただきます。
•アレルギー対応については、事前にご相談・ご連絡ください。
・医師から指導されていること、特に配慮が必要なことがございましたら、事前にお伝え願います。
2. ご利用時間について
・2026年4月27日（月）9:00～18:30
・2026年4月28日（火）9:00～18:30
・2026年4月29日（水/祝）9:30～18:00
　＊当日は5分前から受け入れ可能
・お子様の受け入れ・お引き取りの際には、保護者様の顔写真付きの本人確認書類原本をスタッフにご提示ください。
3. ご提出書類
・託児に関する利用規約及び同意書（本同意書にご記入ください）
・託児利用フォームから保護者様とお子様の情報をご入力ください。
・お子様お預け時に、LINEのお友達登録をお済ませください。
【お預かりについて】
 緊急時はイベント中であっても連絡票に記載の「当日連絡がとれる携帯番号」にご連絡させて頂きます。
 集団保育のため、体調不良のお子様、体温が37.5度以上のお子様はお預かりできません。また、体温が37.5 度未満であっても、下痢や吐の症状がある場合、感染の恐れのある症状がある場合など、保育ができないと判断した場合はお預かりできません。
 万が一の場合には、合同会社ラク育が加入する損害保険で対応させていただきます。但し、保育スタッフの過失以外の原因、不可抗力の場合や体調不良（発熱、嘔吐、下痢などの症状）、天災よる場合はこの限りではありません。
 託児ルーム内でのご利用者様同士のトラブル、保育スタッフの過失以外の原因による事故や負傷等の責任は負いかねます。
 保育スタッフがお子様をお預かりしている時以外の事故や負傷等の責任は負いかねますので、お子様は保護者の責任のもと管理してください。託児運営会社、合同会社ラク育は一切の責任を負いかねます。ご理解の上ご利用いただきますようお願い致します。
◆当日のお預かりについて
・託児スペースに入る前に、お手洗いを済ませてください。
・受付時にお子様の検温を行います。
・託児スペースの消毒、おもちゃの消毒を徹底します。
【持ち物について】
・普段お使いの抱っこ紐がございましたら一緒にお預かりいたします。おもちゃのご用意があり、取り違い・忘れ物を防ぐため、お持ちいただいたおもちゃは、荷物の中にしまっておいてください。
・お荷物はできるだけ一つにまとめてお持ちください。
【食事・おやつについて】
・ 保護者の方でご用意いただいた飲食物のみをお子様に提供します。託児ルームでは食事、飲み物のご用意はございません。
• ご持参いただいた飲食物による食中毒やアレルギー等、万が一お子さまの体調不良等が生じた場合、当社による責任は負いかねますのでご了承下さい。

[Important Points Regarding the Use of Childcare Services]
1. Confirmation of Child's Health Condition
• For the safety and proper care of your child, we will confirm the child’s health condition on the day of service before accepting them into childcare.
• Our service is available for healthy children from 2 months old to elementary school age.
• Please consult and inform us in advance if your child has any allergies.
• If your child requires special considerations or has specific medical instructions from a doctor, please inform us beforehand.

2. Service Hours
• April 27, 2026 (Monday) from 9:00 AM to 6:30 PM
• April 28, 2026 (Tuesday) from 9:00 AM to 6:30 PM
• April 29, 2026 (Wednesday) from 9:30 AM to 6:00 PM
*Children can be accepted starting 5 minutes before the scheduled time.
• When dropping off or picking up your child, please present a valid ID with a photo of the guardian to the staff.

3. Required Documents
• Please fill out the Terms of Use and Consent Form for childcare services (this consent form).
• Enter the necessary information for both the guardian and the child through the childcare service form.
• Please complete the LINE friend registration when dropping off your child.

[Childcare Service Terms]
• In case of an emergency, we will contact the mobile number listed on the "emergency contact" section, even during the event.
• Due to group care policies, we cannot accept children who are unwell or have a temperature above 37.5°C. Even if the temperature is below 37.5°C, we may refuse care if they have symptoms such as diarrhea, vomiting, or other infectious symptoms.
• In case of an accident, Rakuiiku LLC is covered by insurance. However, this coverage does not apply to cases of negligence by childcare staff, force majeure, or health issues (such as fever, vomiting, diarrhea, etc.), or natural disasters.
• We are not responsible for any disputes between users in the childcare room or accidents or injuries caused by factors other than the negligence of childcare staff.
• We are not liable for any accidents or injuries that occur when childcare staff are not directly supervising your child. Please ensure that the child is managed under the guardian's responsibility. Rakuiiku LLC is not liable for any incidents under these circumstances. Please understand and use our services accordingly.

[On the Day of Care]
• Please ensure your child uses the restroom before entering the childcare space.
• We will take your child’s temperature during check-in.
• We will thoroughly disinfect the childcare space and toys.

[What to Bring]
• If you use a baby carrier, we will store it for you. To prevent mix-ups or lost items, please keep any toys your child brings inside their bag.
• Please consolidate your child's belongings into a single bag as much as possible.

[Meals and Snacks]
• Only food and drinks provided by the guardian will be offered to the child. We do not provide meals or drinks in the childcare room.
• We are not responsible for any food poisoning, allergic reactions, or health issues caused by food or drinks brought by the guardian.
